Student Registration Form

This form is to be completed and returned to the NBBEN prior to engagement in ANY of the NBBEN activities. Included in the form is an Authority to Publish component. If any of your students do not wish to be photographed for the specific use of promoting the NBBEN, please notify the relevant NBBEN staff member upon submitting the forms.

Student Evaluation Form

This evaluation form is to be submitted upon completion of Work Placement. The information is used to continually revise the listing of employers and assess their contribution to the program from a student’s perspective. These forms are collated and tallied, with the best performing students and employers receiving awards at the Structured Workplace Learning Awards Night.

Student Own Placement Form

This form is required to be completed if a student would prefer to secure their own placement (not on the NBBEN placement list). This may be a family contact or just an employer of interest. It is required to be completed and returned to the NBBEN at least 5 weeks prior to the placement date. The placement will only be approved after a program induction and an assessment of the employer’s suitability.

Cancellation Variation Request Form

This form is required when any changes are made to confirmed placements. Placements are considered confirmed when the school has received placement summaries and individual student profiles for work placement.
